We welcome referrals from support coordinators, allied health professionals, families and carers. Our team will be in touch promptly to discuss how we can help.
Who can make a referral?
Referrals are welcome from anyone involved in a participant's care — including support coordinators, occupational therapists, social workers, GPs, family members and carers. We work collaboratively with all stakeholders to ensure a smooth transition into our services.

We'll be in touch
Our team will contact you within 1–2 business days to discuss the referral and next steps.
Meet the participant
We'll arrange a meet-and-greet with the participant and their support network to understand their goals.
Support begins
Once a service agreement is in place, we get started delivering tailored, person-centred support.
